Cash Pay / Out-of-Network Services

Revamp Physical Therapy & Wellness is a cash-based practice and is not contracted or in-network with insurance providers.
Payment is due at the time of service.

For clients with PPO insurance plans, a detailed superbill can be provided upon request for possible out-of-network reimbursement directly through your insurance carrier. Reimbursement is not guaranteed and depends on your individual insurance plan benefits.

We do not accept HMO plans

We do not submit insurance claims on your behalf

Clients are responsible for understanding their own out-of-network benefits and coverage
You are also entitled to receive a Good Faith Estimate explaining the expected cost of services in accordance with federal guidelines.

Medicare Information:

Revamp Physical Therapy & Wellness is not enrolled as a Medicare provider and is therefore unable to bill Medicare Part B for covered physical therapy services.
Â
Under federal regulations, physical therapists are generally not permitted to accept private cash payment for Medicare-covered physical therapy treatment services from Medicare beneficiaries.
Â
If you have active Medicare benefits and require medically necessary covered physical therapy services, we will gladly help refer you to a clinic that participates with Medicare.Â
However, Medicare beneficiaries may still be seen for:

Wellness-focused services

Ergonomic consultations

Home safety consultations
-
Non-covered wellness and movement support services
Individuals who have completed formal rehabilitation or insurance-covered therapy services may choose to continue with private wellness-focused services to support ongoing movement, strength, mobility, balance, and overall well-being.

Scheduling & Timing
- Appointments are scheduled at your convenience for in-home or virtual care.
- Please be ready 5 minutes before your visit start time.
- In-home visits require a clear, safe space for treatment.
Safety Protocols
- Clinicians follow strict hygiene and sanitization guidelines for all equipment.
- Privacy is prioritized for both in-home and encrypted virtual platforms.
- Please notify us of any household illness prior to in-home visits.

Cancellations
- We require 24-hour notice for all cancellations or rescheduling.
- Late cancellations may be subject to a standard service fee.
- Notification via phone or email is accepted.
